Chronic Disease Management

Patients who have a diagnosis of a long term condition such as COPD, Asthma, Heart Disease, Chronic Kidney Disease, Learning Disabilities, Epilepsy, Cancer, Hypothyroidism, Mental Health etc will be offered an annual review with a member of our nursing team or GP to review any changes and offer advice and support. Patients will be asked to attend for blood test (where relevant ) and an annual review usually in their birthday month.  If you have more than one long term condition we will aim to review all conditions at the one appointment.

Cervical Smear Test

Women aged 25-64 are eligible for a smear as part of the national programme.  Routine re-calls are every three years for 25-49 year olds and every five years for 50-64 years olds. Contact the practice to book an appointment when you have received your invite letter.

Minor Surgery

Your own doctor can perform certain surgical procedures here at the surgery.  Your doctor will arrange these appointments.

Family Planning and Reproductive Health Care Services

This service is not just about contraception but is about how people look after themselves during their reproductive years, including pre-conception and during and after the menopause.  A full range of services is provided including advice regarding modern family planning techniques.

Counselling

We have a wide range of counselling services available.  Appointments are made by referral from your doctor.

Immunisations

If you have any worries or queries about children’s vaccinations, please feel free to discuss them with your doctor, practice nurse or health visitor.  We strongly recommend that all babies and children are fully immunised.  All staff including nurses have been fully trained in immunisation procedures.

Travel Vaccinations

Please ask at reception for a travel vaccination form which must be completed and handed in at least eight weeks in advance of your holiday.  This information will be passed to one of our practice nurses to process, you will then be contacted to arrange a suitable appointment. A charge may be made for certain immunisations which are not covered by the NHS. Please ask at reception for details.

Flu Vaccinations

An annual flu vaccination is strongly recommended for patients with heart, lung or kidney disease, diabetes or asthma, and all patients aged 65 or over.
