Register with our practice

If you live within our practice area you are welcome to register with us. You can register by:

  • Or contact our reception staff who will be happy to guide you through the procedure.

If you do not register on line you will need to complete a registration form PRF1 and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Please note we are part of the NHS Summary Care Records Service, for further information see the Summary Care Record section on our website.

All new patients can request a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.

Registering Your New Baby

Please register your new baby as soon as possible so we can arrange new baby checks and immunisation appointments.

Named GP

From 1st April 2015, GP Practices are required to allocate every patient with a named GP. This does not affect your treatment or care at the Practice and you can continue to see any GP of your choice as normal.

If you are unsure of whom your named doctor is please ask a receptionists.
